1 Epidemiology of Respiratory Infections

Common colds, flu-like infections, and true influenza are, alongside gastrointestinal infections, the most frequent infections. Approximately 200 different virus types are responsible for upper respiratory tract infections. Adults experience approximately 2—5 upper respiratory infections per year; school-age children are estimated to have 7—10 colds annually (Johnston and Holgate 1996; Eccles 2005).

The classic transmission routes are droplet infections. Bacterial infections frequently follow viral infections due to impairment of the mucous membrane by the viral infection. Fungal infections occur only in immunocompromised individuals (Puta et al. 2026).

It is estimated that acute respiratory tract infections (ARTI) are caused by viruses in over 80% of cases. The most common viruses infecting athletes are the same as those in the general population and include rhinoviruses, non-SARS coronaviruses, influenza A and B, and respiratory syncytial virus (RSV) (Schwellnus et al. 2022).

1.1 Historical Context of Exercise Immunology Research

Exercise immunology emerged as a distinct scientific discipline in the late twentieth century, with approximately 90% of all publications appearing after 1990 (Nieman and Wentz 2019). However, the earliest observations date to 1902, when Larrabee noted that white blood cell differential counts in Boston Marathon runners paralleled those seen in certain disease states (Larrabee 1902).

Four broad research periods can be distinguished: (1) 1900—1979, characterised by basic immune cell counts in exercising subjects; (2) 1980—1989, with the advent of flow cytometry and the first systematic investigations linking exercise to acute respiratory illness; (3) 1990—2009, when the field expanded to include nutritional immunology, exercise and aging, and inflammatory cytokine responses; and (4) 2010 to present, marked by multi-omics approaches, metabolomics, and personalised exercise immunology (Nieman and Wentz 2019).

Notably, a 1984 review published in JAMA in preparation for the Los Angeles Olympic Games concluded that there was “no clear experimental or clinical evidence that exercise will alter the frequency or severity of human infections” (Simon 1984) — a conclusion that motivated the subsequent decades of targeted research that now provides a far more nuanced picture.

2 Symptom Characterization: Early and Late Symptoms

Sneezing reaches its highest values within the first two days after infection and then declines. Coughing, in contrast, increases continuously and can persist for up to 3 weeks. Additional symptoms include: nasal congestion, sinus pain, watery eyes, and headaches. Approximately 50% of affected individuals exhibit muscle and joint pain (Puta et al. 2026; Barrett et al. 2002, 2005).

2.1 Detailed Organ-Specific Symptom Catalogue

A comprehensive organ-by-organ classification of URTI symptoms aids differential diagnosis and supports the “neck check” rule used in return-to-sport decisions (Puta et al. 2026, Tab. 22.5):

This level of clinical detail is essential for sports medicine practitioners making training and competition decisions. Knowing these symptoms by body region helps differentiate URTI from other conditions such as allergies, sinusitis, or early manifestations of systemic infection.

3 Cytokines and Acute Phase Response

The proinflammatory cytokines IL-1 and IL-6 are most significant for fever. Cytokines can cross the blood-brain barrier and, in interaction with the vagus nerve, influence temperature regulation via the hypothalamus. Psychological effects include malaise, attention deficits, mood changes, and altered social interaction (Leon 2002; Mahoney and Ball 2002; Smith et al. 1998).

3.1 Immunometabolism: The Exercise-Induced Perturbation in Metabolites

In athletes exercising intensely for more than 2 hours, significant increases in at least 300 identified metabolites can be measured as body glycogen stores are depleted. There is an extensive increase in numerous and varied lipid superpathway metabolites, including oxidised derivatives called oxylipins (Nieman and Wentz 2019).

Exercise-induced muscle tissue injury and inflammation elicit a strong innate immune response involving granulocytes, monocytes, and macrophages. Immune-specific proteins are produced to regulate the innate immune response, with oxylipins playing a key role in initiating, mediating, and resolving this process. Most of the expressed immune-related proteins — including lysozyme C, neutrophil elastase, defensin 1, proteins S100-A8/A12, cathelicidin antimicrobial peptide, α-actinin-1, and profilin-1 — are involved with pathogen defense, immune cell chemotaxis, and locomotion (Nieman and Wentz 2019; Markworth et al. 2016).

This profound, exercise-induced perturbation in metabolites, lipid mediators, and proteins more than likely has a direct influence on immune function, decreasing the capacity of immune cells to increase oxygen consumption rates after activation. This concept is termed immunometabolism (Nieman and Wentz 2019).

3.2 Immune Cell Metabolic Capacity and Transient Dysfunction

In response to an immunological challenge such as exercise stress, immune cells must engage in growth and proliferation to generate effector cells that produce specific molecules such as cytokines and proteins. This immune activation is associated with substantial oxygen and biosynthetic demands. Immune cells must engage in metabolic reprogramming to generate sufficient energy (Nieman and Wentz 2019).

Preliminary data support that immune cell metabolic capacity is decreased during recovery from physiologically demanding bouts of intensive exercise, resulting in transient immune dysfunction. Immunonutrition support — especially increased intake of carbohydrate and polyphenols — has been shown to counter these exercise-induced decrements in immune cell metabolic capacity (Nieman and Wentz 2019; Nieman et al. 2018).

4 Respiratory Infections and Competitive Sports

Respiratory infections represent, alongside injuries, the most frequent cause of training and competition absences in competitive sports. Acute respiratory tract infections (ARTI) are a common reason for physician visits among athletes — during Olympic competitions, they account for approximately 50% of medical consultations. The overall incidence is approximately 1.8 infections per athlete per year with an average symptom duration of 7.1 days (Schwellnus et al. 2022; Snyders et al. 2022).

4.1 Epidemiological Evidence from Major Sporting Events

A substantial body of epidemiological research has established the link between heavy exertion and elevated URTI risk in athletes (Nieman and Wentz 2019):

The IOC and IAAF initiated acute illness surveillance systems revealing that 2—18% of elite athletes experience illness episodes during international competition events, with higher proportions for females and those engaging in endurance events (Schwellnus et al. 2022; Nieman and Wentz 2019).

4.2 Risk Factors for Illness in Athletes

Numerous factors beyond training load modulate infection risk in athletes. These can be categorised based on Puta et al. (2026, Tab. 22.3) and Nieman and Wentz (2019):

  • Insufficient physical and psychological recovery
  • Excessive training volume, duration, and intensity
  • Too frequent high-intensity anaerobic training (e.g. intensive interval training)
  • Infections, premature return to sport after infections, allergies
  • Altitude training camps
  • Psychological distress
  • Malnutrition or unbalanced nutrition before, during, and after training
  • Insufficient sleep duration, sleep quality, and irregularity
  • Insufficient consideration of the female menstrual cycle in training load management
  • Female sex
  • High levels of depression or anxiety
  • International travel across several time zones
  • Participation in competitive events especially during winter months
  • Rapid weight loss and crash diets

The mismatch between current load and current load capacity is a key determinant. The subjective assessment by athlete, coach, and supervising physician regarding the scope and intensity of training loads must consider changing circumstances such as health status (Puta et al. 2026).

4.3 Performance Consequences of URTI in Athletes

The decrease in exercise performance after a URTI can last 2—4 days. Runners who start an endurance race despite systemic URTI symptoms are 2—3 times less likely to complete the race (Nieman and Wentz 2019). Paralympic athletes have unique pre-existing medical conditions that predispose them to an increased risk of illness, with incidence rates of 10.0—13.2 episodes per 1000 athlete-days in Summer and 18.7 per 1000 athlete-days in Winter Paralympic Games (Nieman and Wentz 2019).

Since the emergence of SARS-CoV-2 in late 2019, it has been documented that infection with SARS-CoV-2, compared with a common ARTI, is associated with more symptoms during the acute illness as well as a significantly longer time span until return to play (on average 30 days for SARS-CoV-2 vs. 10 days for ARTI in general; Hull et al. 2022; Schwellnus et al. 2021). Krzywanski et al. (2022) reported that 88% of athletes infected with SARS-CoV-2 were impaired in their training and lost an average of 8.1 training days.

4.4 Return to Sport After Mild/Moderate COVID-19: Recommendations and Immunological Rationale

Since the initial emergence of SARS-CoV-2, the relationship between acute respiratory infection and sport has gained a new dimension of clinical relevance. Even though widespread acquired immunity has reached an endemic state, the continued evolution of variants means that understanding the immunological time course remains essential for sports medicine practitioners (Puta et al. 2026; Schwellnus et al. 2022).

Characterisation of symptoms and immunological response to COVID-19

The immunological response to SARS-CoV-2 infection follows a characteristic temporal sequence (Puta et al. 2026; Sette and Crotty 2021; Tay et al. 2020):

Minutes to approximately 2 days after infection (before symptom onset): SARS-CoV-2 initiates infection by binding its spike protein’s receptor-binding domain to the angiotensin-converting enzyme 2 (ACE2) receptor on host cells. The early innate immune response involves activation of neutrophils and monocytes through recognition of pathogen-associated or damage-associated molecular patterns within hours of infection. Resident macrophages, epithelial cells, and infiltrating cells of the innate immune system simultaneously produce chemokines and proinflammatory cytokines (IL-6, TNF, IL-1, IFNs) to create an antiviral environment and recruit additional leukocytes to the site of infection (Sette and Crotty 2021; Hope and Bradley 2021; Tay et al. 2020). The systemic rise in proinflammatory cytokine levels is responsible for the induction of “sickness behaviour” and the typical symptoms such as fever, headache, and myalgia that emerge in the first days of COVID-19 (Kelley et al. 2003; Schwellnus et al. 2022).

The acute adaptive effector response (up to approximately 10 days after symptom onset): The first SARS-CoV-2-specific helper T cells (CD4⁺) can be detected 2—4 days after symptom onset, accompanied by a somewhat later pronounced cytotoxic T cell response (CD8⁺) (Sette and Crotty 2021). B cell expansion takes longer, with IgM and IgA antibodies first detectable 5—7 days after symptom onset and IgG antibodies 7—10 days after symptom onset. By days 6—10, the number of adaptive effector cells has reached a level sufficient to control the viral infection, with the viral load peaking around days 5—6 and approximately 90% of individuals seroconverting by day 10 through orchestrated T and B cell responses (Tay et al. 2020; Sette and Crotty 2021). Notably, a rapid SARS-CoV-2-specific CD4⁺ T cell response in acute COVID-19 has been associated with mild illness and accelerated viral clearance, whereas a conspicuous prolonged absence of SARS-CoV-2-specific CD4⁺ T cells has been associated with severe or fatal COVID-19 (Braun et al. 2020; Tan et al. 2021).

Immunological memory: Following viral elimination, a small number of T cells form memory T cell pools that peak within 2 weeks but remain detectable more than 100 days after infection (Hope and Bradley 2021). B cells similarly differentiate into memory cells and long-lived plasma cells. This virus-specific immunological memory facilitates faster viral clearance upon second antigen contact. Accordingly, reinfections or breakthrough infections tend to be associated with milder illness and better clinical outcomes, and the clinical course of disease is generally milder in individuals who have acquired immunity through vaccination (Puhach et al. 2023; Hope and Bradley 2021).

Return to sport after SARS-CoV-2 infection — current recommendations

Athletes with mild to moderate COVID-19 symptoms should observe a training pause until complete clinical recovery and symptom resolution (Burgstahler and Niess 2021; Steinacker et al. 2022). The consensus statement by Steinacker et al. (2022), based on 18 synthesised articles and expert opinion, recommends the following graduated approach:

  • Mild illness: Training may resume after 3 consecutive symptom-free days.
  • Asymptomatic infection: Training should be paused for at least 3 days after diagnosis.
  • Competition return: Competitions should not be resumed until at least 10 symptom-free days have passed and the athlete is free of complaints and tolerating progressive load increases (Steinacker et al. 2022).

Elliott et al. (2022) have further stratified these recommendations based on symptom presentation. In particular, athletes with symptoms of the lower airways (“below the neck”) are recommended an initial rest period of at least 5 days, whereas athletes with symptoms confined to the upper airways (“above the neck”) may continue light activities without complete exercise cessation (Elliott et al. 2022).

From an immunobiological perspective, it appears advisable to find a balance between previously recommended longer rest periods and newer, more abbreviated approaches. Considering the important time windows of acute innate and adaptive immune infection regulation, resuming activities after 10 days of rest, followed by at least 3 consecutive symptom-free days, appears to be a prudent approach (Puta et al. 2026; Haunhorst et al. 2023a).

This conservative recommendation is supported by several considerations: resumption of sporting activities during the acute infection regulation period could jeopardise viral clearance, the resolution of inflammation, and long-term athletic performance (Haunhorst et al. 2023b). Exercise-induced redistribution of effector lymphocytes mediated by catecholamine signalling, and energy allocation to working musculature via IL-6 release, could be relevant in this context (Campbell and Turner 2018; Kistner et al. 2022). Furthermore, silent hypoxia has been reported as a common feature of acute COVID-19, attributable to renin-angiotensin system dysfunction following ACE2 binding by the spike protein. Additional oxygen deficit through intensive physical exertion may consequently lead to cardiopulmonary complications (Fajloun et al. 2023).

Clinical staging and medical investigations

The return to training and competition should be staged according to WHO guidelines as asymptomatic, mild, moderate, severe, and critical (Steinacker et al. 2022; NIH 2024):

Elliott et al. (2020, 2022) recommend that athletes with a complicated clinical course should undergo monitoring of inflammatory markers (e.g. C-reactive protein), respiratory function assessment (spirometry), and renal function evaluation. Steinacker et al. (2022) add that pulmonary function tests should be performed when clinical findings point to bronchial constriction or dyspnoea at rest or during exertion. Medical examinations should be considered for athletes wishing to return to competition earlier than 10 days after symptom onset, and should include a medical history, physical examination, laboratory diagnostics (differential blood count, CRP, transaminases, CK, creatinine, and troponin if cardiac complaints are present), and a resting ECG (Steinacker et al. 2022). Myocardial or pulmonary conditions may even require regular annual follow-up including ECG, echocardiography, and arterial blood gas analysis (Halle et al. 2021).

Post-COVID condition: special considerations for return to sport

Persistent cardiac symptoms during the medical evaluation process should prompt further investigations (Gluckman et al. 2022). If none of the investigations yields abnormal results and no other symptoms persist beyond 4 weeks after symptom onset, one of the described graduated return-to-sport protocols may be applied. Persistent symptoms suggestive of a post-COVID condition — such as acute exercise intolerance and post-exertional symptom exacerbation (PEM) — can represent a challenge for general physical activities and require specialised medical and sports-scientific supervision (Zhang et al. 2025; Petek et al. 2022). A recent study showed that elite athletes with post-COVID condition experienced reduced quality of life, poorer competitive performance levels, and altered training parameters, independent of sex, and that mild ongoing inflammation based on CRP levels was associated with self-reported cardiac and flu-like symptoms (Zhang et al. 2025). Latest findings suggest that the pathophysiological basis may involve tissue hypoxia and hypoperfusion due to microcirculatory disturbances, microcoagulation, morphological changes in red blood cells, vasoactive autoantibodies, and persistent immune activation (Altmann et al. 2023; Haunhorst et al. 2022; Haunhorst et al. 2024). Based on current knowledge and experience with myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS), no progressive training programmes can currently be recommended for post-COVID condition (Puta et al. 2026).

5 Infection Foci as a Cause of Infection Susceptibility

In addition to allergies, infection foci can be a cause of athletes’ susceptibility to infection. The search for infection foci in the area of the teeth or the upper airways is an ever-recurring task in sports medicine practice. Besides obvious and visible entry-point infections, other infectious causes in the dental area should be clarified through dental examinations. In addition to sinus infections (which can often only be diagnosed through specialist examinations), chronic tonsillitis with purulent discharge and lymph node swelling in the submandibular region represents a relatively frequent source of infection foci (Puta et al. 2026).

Particular attention should be paid to potential organ-related secondary diseases (heart, kidneys, liver, spleen) to make vital diagnoses such as myocarditis in a timely manner (Puta et al. 2026).

6 The J-Curve Model: Infection Frequency and the Exercise Workload Continuum

The majority of infections in athletes are so-called banal infections with irritations in the nasopharynx, runny nose, impaired nasal breathing, and mild sore throat. General symptoms such as limb pain, chills, and fever generally do not occur (Puta et al. 2026).

6.1 The J-Curve Model (Nieman 1994)

To illustrate the relationship between sport and upper respiratory tract infection frequency, the model of the “J-shaped” curve is used (Nieman 1994; Puta et al. 2026). This model proposes three strata along the exercise workload continuum:

  • Sedentary individuals (too little activity): medium/average risk of URTI.
  • Moderate, individually appropriate exercise: lowest risk of infection — the protective “sweet spot.” Athletes with a moderate, individually non-overtaxing training volume have a lower risk of illness.
  • Heavy/excessive exercise (too much): highest risk. Overtrained athletes and those performing single exhaustive bouts carry the highest risk.

The model also illustrates the inverse relationship between immunity and infection frequency: as physical activity increases from sedentary to moderate levels, immunity rises and infection frequency drops. Beyond the moderate range into excessive exercise, immunity declines and infection frequency rises sharply. This dual visualisation was termed the “Immunfisch” (immune fish) by Puta et al. (2026, Abb. 22.7) because the combined curves resemble the outline of a fish.

6.2 Supporting Epidemiological Evidence

The J-curve model is supported by evidence from both randomised controlled trials and prospective epidemiological studies (Nieman and Wentz 2019):

Randomised controlled trials demonstrating that moderate exercise reduces URTI:

  • Nieman et al. (1990a): 36 mildly obese sedentary women randomised to 15 weeks of moderate walking (45 min/day × 5 days/week) experienced fewer days with illness symptoms (5.1 ± 1.2 days) compared to controls (10.8 ± 2.3 days).
  • Chubak et al. (2006): 115 postmenopausal women randomised to 1 year of moderate exercise showed an illness incidence of 30% vs. 48% in controls, with a three-fold decreased risk in the final 3 months.
  • Barrett et al. (2012, 2018): 373 older adults randomised to 8 weeks of moderate sustained exercise demonstrated proportional reductions in URTI incidence (14%), days-of-illness (23%), and global severity (31%) compared with controls.

Collectively, these randomised clinical trials (8 weeks to 1 year in length) demonstrate that moderate exercise programmes reduce URTI incidence and duration, with magnitudes of reduction (typically 40—50%) that exceed those reported for most medications and supplements (Nieman and Wentz 2019).

Epidemiological studies confirming the protective effect of moderate activity:

  • Nieman et al. (2011): 1,002 adults followed for 12 weeks — the number of days with URTI was 43% lower in those exercising ≥5 days/week (≥20 min aerobic exercise) compared to sedentary individuals (≤1 day/week), and 46% lower when comparing highest vs. lowest fitness tertile.
  • Weighted mean across studies: 28% reduced URTI rates in high vs. low physical activity and fitness groups (Nieman and Wentz 2019).

6.3 Critical Appraisal of the J-Curve Model

Several important caveats apply to the J-curve model (Puta et al. 2026; Nieman and Wentz 2019):

  • The IOC consensus group (Schwellnus et al. 2016) provided support for the J-curve model but cautioned that the right side of the model may not apply to elite athletes at the highest level, where high training loads are not consistently associated with an increased risk of illness.
  • Elite athletes may perceive (and report) mild infection symptoms more readily than untrained individuals, introducing reporting bias in questionnaire-based studies (Brenner et al. 1994).
  • Beyond training volume and intensity, numerous other factors modulate the immune system and must be considered (see the comprehensive risk factor list in Section 2.4.2).
  • Individual cases can deviate from the general trend. The J-curve represents a population-level didactic model, not an individual prediction tool (Puta et al. 2026).
  • The direct connection between exercise-induced immune changes and actual infection risk has not yet been fully established and will require long-term studies with large cohorts using clinically confirmed infections rather than self-reported symptoms (Nieman and Wentz 2019; Simpson et al. 2020).

Nevertheless, the J-curve illustrates the relationship between sport and upper respiratory tract infections in its basic tendency and represents a didactically effective model (Puta et al. 2026).

7 Clinical Benefits of the Exercise-Immune Relationship

7.1 Reduced Systemic Inflammation in Physically Active and Lean Individuals

Each exercise bout causes transient increases in total white blood cells, granulocyte-related proteins, and a variety of plasma cytokines including IL-6, IL-8, IL-10, IL-18, IL-1 receptor antagonist, granulocyte colony stimulating factor, and monocyte chemoattractant protein 1. Over time, the summation of these transient responses produces an anti-inflammatory phenotype (Nieman and Wentz 2019).

Physically fit individuals have lower resting levels of inflammatory biomarkers: serum CRP shows a 4.4-fold difference between obese individuals (mean BMI 31.7) and endurance athletes (mean BMI 23.0); plasma IL-6 shows a 1.3-fold difference (Nieman and Wentz 2019). The upper tertiles of fitness and exercise frequency are associated with reduced numbers of days with URTI: a 46% reduction for the highest vs. lowest fitness tertile and a 43% reduction for the highest vs. lowest exercise frequency tertile (Nieman et al. 2011).

The persistent increase in inflammation biomarkers in physically inactive individuals is defined as chronic or systemic inflammation and is linked with multiple disorders and diseases including obesity, arthritis, atherosclerosis, cardiovascular disease, chronic kidney disease, liver disease, metabolic syndrome, insulin resistance, type 2 diabetes mellitus, sarcopenia, osteoporosis, and various types of cancers (Nieman and Wentz 2019; Gleeson et al. 2011).

7.2 Exercise-Immune Linkages to Reduced Chronic Disease

There is increasing evidence that the circulation surge in cells of the innate immune system with each exercise bout, together with the anti-inflammatory and antioxidant effect of exercise training, has a summation effect over time in modulating tumorigenesis, atherosclerosis, and other disease processes. Exercise training counters the disease process by stimulating many cellular and molecular changes throughout body tissues that promote anti-inflammatory and antioxidant responses and augment immunosurveillance (Nieman and Wentz 2019; Fiuza-Luces et al. 2023).

7.3 Exercise, Gut Function, and the Microbiome

The gut microbiota influences human health and immune function, in part through the fermentation of indigestible food components in the large intestine. Recent studies indicate that exercise and physical fitness diversify the gut microbiota, enhancing the number of benign microbial communities. More human research is needed to establish whether the positive linkage between long-term exercise training and a diverse microbiome translates to improved immune function (Nieman and Wentz 2019).

8 Nutritional Countermeasures to Exercise-Induced Immune Changes

8.1 Carbohydrates Attenuate Post-Exercise Inflammation

A consistent finding is that carbohydrate intake during prolonged and intense exercise (whether from 6—8% beverages or sugar-dense fruits such as bananas) is associated with higher blood glucose and insulin levels, lower plasma stress hormones (epinephrine and cortisol), diminished fatty acid mobilisation and oxidation, and reduced inflammation as measured by a variety of biomarkers including skeletal muscle IL-6 and IL-8 mRNA, blood neutrophil and monocyte cell counts, and cytokines such as IL-6, IL-1ra, and IL-10 (Nieman and Wentz 2019; Nieman et al. 1998a, 1998b).

A carbohydrate intake of at least 30 g per hour can attenuate the rise in cortisol and adrenaline concentration during prolonged endurance exercise. In 2.5-hour runs, placebo-controlled comparisons showed that substitution with a 6% carbohydrate beverage significantly reduced the acute inflammatory response, with particular attenuation of interleukin-6, which plays a central role in the acute inflammatory response to exercise (Nieman et al. 1998a, 1998b; Puta et al. 2026). The effect of carbohydrate ingestion in attenuating post-exercise inflammation is strong (approximately 30—40%), especially when contrasted with water-only intake in overnight-fasted athletes (Nieman and Wentz 2019).

The stress hormone response (cortisol, epinephrine, norepinephrine) to prolonged continuous endurance activity is dependent on carbohydrate substitution, as demonstrated by a systematic review by Christ et al. (2024). This represents an effective nutritional strategy before and during exercise to attenuate the rise in cortisol and adrenaline levels (Puta et al. 2026; Christ et al. 2024).

8.2 Polyphenols Counter Exercise-Induced Immune Changes

Polyphenols, in particular flavonoids, have attracted attention owing to their bioactivity and related health benefits. Gut-derived phenolics circulate throughout the body after increased polyphenol intake, exerting a variety of bioactive effects that are important to athletes including anti-inflammatory, antiviral, antioxidative, and immune cell signalling effects (Nieman and Wentz 2019).

Evolving data support the intake of fruits such as dates, raisins, and bananas by athletes during training to provide the sugars and polyphenols that exert anti-inflammatory influences (Nieman and Wentz 2019; Nieman et al. 2018).

8.3 Micronutrients and Trace Elements

Iron occupies a special role among minerals due to the ease of detecting deficiency and its clinical relevance. Endurance athletes — especially adolescent and young adult females — are most at risk for developing manifest iron deficiency. A laboratory screen is recommended at regular intervals, at minimum annually, for preventive reasons (Haymes and Rebstock 1989; Puta et al. 2026).

Regarding trace elements important for immune defence (selenium, zinc, copper), establishing valid correlations between infection frequency and supplementation is currently difficult. A total intake within the recommended dietary guidelines of the Deutsche Gesellschaft für Ernährung (DGE 1997) can be considered safe. Zinc acetate lozenges at doses up to 75 mg/day have been shown to positively influence the course of beginning cold symptoms, though no data specific to the intersection of infection and sport currently exist (Hemila et al. 2017; Puta et al. 2026).

A general supplementation with antioxidant vitamins for athletes cannot be recommended with adequate energy availability. In problematic cases, substitution with antioxidant vitamins (Pro-Vitamin A, Vitamin C, Vitamin E) during infection-prone seasons may be considered (Puta et al. 2026).

9 Strategies for Athletes to Minimise the Risk of Impaired Immune Function

The following strategies are derived from Puta et al. (2026, Sections 22.6—22.7) and Nieman and Wentz (2019) and are designed to help reduce the impairment of immune function:

9.1 Training and Competition Load Management

  1. Avoid very long training sessions (>2 h).
  2. Reduce psychological stress and develop appropriate coping strategies.
  3. Monitor mood, stress, and anxiety.
  4. Knowledge of the relationship between exercise intensity, volume, and activation of the sympathetic nervous system and the hypothalamic-pituitary-adrenal axis. Factors that strongly influence the stress hormone response include: fasting, low glycogen stores, dehydration, heat, cold, altitude, psychological stress, and sleep deficit.
  5. Ensure adequate sleep duration (~8 h) and sleep regularity.
  6. Avoid crash diets and rapid weight loss.
  7. Avoid/minimise contact with sick persons (adults and children).
  8. Knowledge and implementation of hygiene rules (especially oral hygiene 2× daily) and disinfection techniques (hand disinfection).
  9. Do not share drinking bottles, towels, cutlery, etc.
  10. Avoid touching hands in the nasal and oral area.
  11. Knowledge of warning signs and symptoms of infections of the upper airways and of overtraining.

9.2 Nutritional Strategies for Infection-Prone Athletes

A thorough nutritional counselling for infection-prone athletes should be conducted to identify and eliminate one-sided or unfavourable dietary patterns. Nutritional protocols (analogue or digital) can be used to gain insight into potential weaknesses in nutrition in the context of exercise, training, and sport (Puta et al. 2026).

The most targeted measure for promoting immunity is active vaccination. Vaccination status should be reviewed according to generally accepted criteria, including for foreign travel. Passive immunisation through administration of immunoglobulins should generally be preferred over active vaccination (Puta et al. 2026).

Consume a well-balanced diet with sufficient energy to maintain a healthy weight, with a focus on grains, fruits, and vegetables to provide sufficient carbohydrate and polyphenols. Consider the knowledge that when taking into account the necessary recovery times and appropriate structuring of subsequent training sessions and a suitable nutritional strategy, no increased infection susceptibility results (Nieman and Wentz 2019; Puta et al. 2026).
